Admissions - Connective Healthcare Academy LLC

Potential applicants must contact Connective Healthcare Academy LLC by visiting the institution and meeting with an admissions representative, who will give prospective students a tour of the campus, provide detailed information on the school's programs and policies, discuss applicants' qualifications and assist them to determine the best way to meet their career objectives. Admission to every training program and seminar is dependent upon attending and passing an interview with the school director. 

To ensure a smooth and successful admission process, please review and complete the following requirements:

1. Be at least 18 years of age
2. Provide a copy of negative TB PPD on first day of class (depending on the requirements of the clinical site).
3. Criminal History Check- This is necessary to ensure the safety and well-being of our students and patients.
4. Nurse Aide Registry Verification- which confirms your eligibility to participate in our CNA program.
5. Complete Misconduct Registry Check- This check is required to verify that you have not been listed in any misconduct registries.
6. Application Deadline-Ensure that your application and all required documents are submitted by the application deadline. Applications received after the deadline may not be processed for the upcoming term.
7. Proof of High School Graduation or Equivalent
8. valid form of identification, such as a driver’s license or passport. 
9. Prerequisite Certifications (CBT - Computer-Based Training)-If applicable, submit proof of completion of the computer-based training (CBT) courses required for admission.

All forms and documentation must be completed and submitted by the specified deadline. Incomplete applications may result in delays or denial of admission.
